HealthCare Support is actively seeking a Nurse Practitioner - Float to fill an opening in Hartford County, CT. This role is a 3+ month contract position (extensions/perm possible after contract).
Daily Responsibilities for Nurse Practitioner - Float:
Conduct comprehensive assessments, patient facing
Effectively manage acute and chronic medical and behavioral conditions
Ensure accurate and complete documentation, including ICD 10 conditions.
Communicate and collaborate with the interdisciplinary care team
Conduct advanced illness and advanced care planning conversations
Provide patients and caregivers with counseling and education
Travel to three different senior living facilities in the Hartford County area for patient assessments
Working hours are Monday to Friday, 8am to 5pm
Required Qualifications for Nurse Practitioner - Float:
MSN
Active APRN license in Connecticut
1+ year of relevant nurse practitioner experience
Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
Vaccinated from COVID-19
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (reimbursement for the license available)

Pay: $60 - $85 an hour
