Case Manager

HealthCare Support is actively seeking a Case Manager to fill an opening with a nationally ranked healthcare provider in Golden Valley, MN.

Daily Responsibilities for Case Manager:

Create discharge plans, participate in the referral process, and perform utilization review for critically ill patients

Shift: Monday-Friday 7:30/8:00 am-4:30/5:00 pm, no weekends, no on call schedule

Required Qualifications for Case Manager:

RN, RT, or SW license in the state of Ohio
Inpatient hospital experience dealing with critical care patients
1+ years of case management/discharge planning experience in the hospital setting

Pay Details: $70,000 - $85,000 (Depending on experience)
