Daily Responsibilities for Field LPN/LSW Patient Care Advocate:
- Bachelor's degree OR 4 years of related experience
- Strongly prefer licensure: LPN, LMSW, C-SW, LSW
- 2+ years of experience in HEDIS and/or Quality Improvement - looking for experience coaching providers on how to improve HEDIS scores
- Understanding of HEDIS measures and quality improvement initiatives
- Good tenure in past positions, 1-2+ years in prior roles
- WFH capability - smart phone, high speed internet, private work space
- Own personal transportation, have active DL and insurance, ability to pass DMV report
- Computer Skills: Strong Excel Skills
