Physical Therapist needed for an assignment at Hampton City Schools, Hampton, Virginia in Hampton, Virginia.
Shift Details: (Days - Mon,Tue,Wed,Thu)
2:00 PM to 7:00 PM
Coverage Dates: 04-Dec-2023 to 29-Feb-2024
2-3 years recent experience
Experience working with different chronic medical conditions and various patient populations is a plus
Provide on-site physical therapy using evidence based therapeutic modalities to help patients heal from acute injury or manage chronic health conditions.
PT is on site, but might provide virtual services from the health center
Develop and monitor adherence to home exercise programs related to a patients musculoskeletal condition.
Assist patients in making lasting changes to improve their overall health using therapeutic interventions.
Be a facilitator to ensure patients continuity of care and serve as a conduit between the services of the health center, collaborating providers, and the community.
Provide education related to injury prevention, safety, ergonomics, chronic disease management through movement strategies, and the role of musculoskeletal wellness in the management of chronic conditions to patients and employers.
Act as the subject matter expert for early identification, treatment, and self-management of musculoskeletal conditions for prevention of unnecessary specialty and radiology referrals
