Staff Hours: (Days - Mon, Tue, Wed, Thu, Fri)

07:00 AM to 6:00 PM

(Days - Tue, Wed)
07:00 AM to 3:00 PM

(Days - Fri)
07:00 AM to 1:00 PM


Requirements:

•Primary care and/or chronic disease management experience
•Must be proficient in the use of an electronic medical record; experience with eClinicalWorks is a plus
•Must have experience seeing children 2+
